• Bachelor’s degree in a Human Services field
• Driver’s license and proof of auto insurance required.

• Provide case management for an assigned group of clients including coordinating and monitoring the activities of the treatment team.
• Assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in conjunction with the treatment team.
• Ensure that necessary individual supportive counseling/therapy and symptom management is performed; ensuring immediate changes are made in the treatment plan, as client’s needs change.
• Act as liaison and consult with community agencies and families to maintain coordination in the treatment process.
• Document client’s progress to maintain a permanent record of client activity according to established methods and procedures.
• Work with vocational specialist within ACT team structure to ensure optimum level of vocational functioning for assigned caseload.
• May provide transportation to clients as needed.
• Perform medication monitoring, administration, and pre-filling services as needed.
