Compensation:$40 - $45 hourly
Responsibilities:
- Provide compassionate, patient-centered care in the comfort of patients' homes, ensuring their safety and well-being.
- Administer medications and treatments as prescribed, monitoring patients' responses and reporting any changes to the healthcare team.
- Collaborate with patients, families, and the healthcare team to develop and implement personalized care plans.
- Educate patients and their families on health management and disease prevention, empowering them to take charge of their health.
- Document all patient interactions and care provided accurately and promptly, maintaining confidentiality and compliance with regulations.
- Conduct regular assessments of patients' health status, adjusting care plans as needed to promote optimal outcomes.
- Communicate effectively with the healthcare team to ensure continuity of care and address any concerns or changes in patient condition promptly.
Qualifications:
- Experience in providing patient-centered care in home health settings.
- Ability to administer medications and treatments accurately and safely.
- Proven track record of collaborating effectively with healthcare teams and families.
- Strong communication skills to educate patients and families on health management.
- Proficiency in documenting patient interactions and maintaining confidentiality.
- Ability to conduct thorough health assessments and adjust care plans accordingly.
- Licensed Practical Nurse (LPN) certification in the state of California required.
