Job Summary:
The Wellness Coordinator assists the Wellness Director in managing health programs and personal care for residents. In addition to administrative and leadership responsibilities, the Coordinator oversees medication ordering, monitoring and administration, and is specifically responsible for TB testing of employees and residents. Treat all residents, guests, and staff in a friendly, personable and professional manner. Provide best practices in customer service, serving as a role model for less-senior staff.
Principal Responsibilities:
I. New Resident Intake
A. Obtain new resident file from Wellness Director and complete admission records as needed:
- 24-Hour Assessment and initial Service Plan
- Vital signs, blood pressure, weight
- Current medication list and allergies
If some of the above have been completed by other staff, review for accuracy, fill in missing elements, and become familiar with resident’s case. Ensure that CNAs are informed of needs documented on initial Service Plan.
B. As assigned, meet with new residents to complete Comprehensive RAI and Service within 14 days of admission. Provide required information about Advance Directives.
II. Incident Response
A. Respond to calls about resident falls, emergencies and other incidents. Assess health status of all people involved and provide first aid or other services as appropriate.
B. Assist with emergency clean-ups, summoning help from housekeeping, maintenance and/or security as needed.
C. Call (or have others call) 911, attending physician, family members, and/or other parties. Report significant incidents and problem behaviors to Wellness Director and/or Executive Director.
D. Chart observations, actions and notifications in electronic progress notes and on incident report.
III. Management of Medications
A. Oversee ordering, set-up, delivery, and administration of residents’ medication. Insure that residents are receiving the proper amount of supervision or assistance, and make changes to the services they receive as needed. When a resident refuses needed services, ensure that s/he signs Negotiated Risk Agreement.
B. Make changes to service plan and/or medication list whenever a resident’s regimen changes.
For residents on ATC packs, add new meds to the packets and remove discontinued meds.
C. Train and supervise CNAs in the proper procedures for med reminders and assistance.
D. Check all pharmacy deliveries for completeness and accuracy; rectify discrepancies immediately.
Process “stat” medication orders with all due diligence. Maintain records of deliveries.
E. Keep Wellness Director informed of medication management issues and the performance of the contracted pharmacies.
IV. Resident Assessments and Record-keeping
A. Perform quarterly assessments and service plan updates as assigned; Involve resident in the process and have him/her sign form; ensure that RN reviews and approves updates.
B. Provide health care counseling and hands-on services to residents as needed. Refer them to on- site clinic, to specialists or to other health care providers as needed.
C. Record all services provided, referrals and changes in resident condition in progress notes.

VI. Staff Supervision
Monitor CNA job performance and record-keeping.
