Role Description
This is a full-time, on-site role for a Hospice RN Case Manager located in Tucson, AZ. The Hospice RN Case Manager will be responsible for providing compassionate, comprehensive care to patients with terminal illnesses. Daily tasks will include managing patient care plans, coordinating with the interdisciplinary team, performing nursing assessments, and providing symptom and pain management. Additionally, the role involves educating patients and their families about hospice services, and ensuring proper utilization management and discharge planning.
Qualifications
- Strong Nursing and Medicine skills
- Experience in Case Management and Discharge Planning
- Proficiency in Utilization Management
- Excellent communication and interpersonal skills
- Ability to handle sensitive and emotional situations with care and professionalism
- Valid RN license and BLS certification
- Previous experience in hospice or palliative care is a plus
