Role Description The Hospice RN Case Manager is a full-time role based in Coeur d'Alene / Sandpoint, ID. This role is responsible for coordinating and managing hospice care plans, including assessing patient needs, developing individualized care plans, and adjusting services as conditions change. Day-to-day tasks include conducting home visits, providing symptom management and education, communicating with patients, families, and caregivers, and collaborating closely with physicians, nurses, social workers, and other interdisciplinary team members. The Hospice Case Manager ensures regulatory compliance, accurate documentation, and timely communication in the medical record and with the care team. This role also participates in care conferences, supports families with end-of-life planning and resources, and promotes a high standard of compassionate, dignified care.

Qualifications

• Clinical expertise in hospice, palliative care, or home health, including assessment, care planning, and symptom management.
• Strong case management skills, with the ability to coordinate interdisciplinary care and manage a caseload efficiently.
• Excellent communication and interpersonal skills for collaborating with patients, families, caregivers, and healthcare professionals.
• Proficient in documentation, electronic medical records, and basic computer skills to ensure accurate and timely record-keeping.
• Ability to work independently in the field and from home, manage time effectively, and prioritize patient needs.
• Current, unrestricted Registered Nurse (RN) license in the state of Idaho (or appropriate clinical license, as required by Eden Health).
• Valid driver's license, reliable transportation, and the ability to travel locally for home visits.
• Previous hospice or home health experience preferred; familiarity with Medicare hospice regulations is an advantage.
• Demonstrated commitment to compassionate end-of-life care and support for diverse patients and families.
