Essential Job Responsibilities:
- Makes visits to clients home to provide nursing care based on the client needs and their care plan. These visits could be scheduled or as needed.
- Assesses the client's condition and reports any concerns to the clients Ecumen RN Case Manager. Collaborates with the RN Case Manager to update the care plan as needed and/or as directed by the medical provider.
- Administers medications and treatments as prescribed by the medical provider, following the plan of care. Completes charting as appropriate for type of visit.
- Communicates with appropriate medical providers regarding client needs and/or changes in the client’s condition. Seeks orders from the medical provider as required.
- Provides health care instructions to the client as appropriate per assessment and care plan.
- Receives and responds to phone calls during assigned shifts.
Registered Nurse HC Minimum Required Qualifications:
- Graduate of an accredited Registered Nursing program
- 1 year of nursing experience in post-acute care setting
- Must be licensed as a Registered Nurse within the state in which working and must maintain licensure.
- Valid Drivers License, proof of insurance and satisfactory motor vehicle record
- Ability to communicate effectively both verbal and written
- Proficient with Microsoft Office Suite, OASIS Documentation
- Able to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Home Care, Bill of Rights, OSHA, Universal Precaution, AWAIR, Right to know, and SDS, ADA HUD policy and procedures, Fair Housing laws and regulations.
- Bachelor of Science degree in Nursing
- One year nursing experience in Home Care
- Public Health Nurse certificate
