Job description
The TLC under direction of the Program Director will be assigned to veterans from the time the veteran enters the program through the time the veteran is discharged. The TLC will act as a liaison between veteran and programs and provide comprehensive case management, engaging veterans and linking them to additional services. The TLC’s role is to assist veterans in strengthening and establishing goals that will facilitate success throughout their lives. The TLC will support personal growth, behavior modification of their clients, and formulate strategies to help them meet their goals.
Job Duties:
- Meet with and evaluate clients
- Provide weekly individual sessions to discuss needs and goals
- Develop goals, strategies, and plans for clients
- Adjust clients’ goal strategies as needed
- Keep records of client progress
- Facilitate compliance with referrals
- Attend appropriate program meetings.
- Attend appropriate trainings and maintain knowledge of community resources.
- Present information obtained at trainings with component staff.
- Work with other staff and community human service providers to ensure an integrated approach and provide comprehensive services to veterans and families.
- Provide information and case management services to families.
- Prepare and maintain required records in client database.
- Coordinate with other service areas to disseminate information to veterans in a timely manner.
- Update information on case loads
- Participate in recruitment activities and enrollment clinics throughout the program year as assigned.
- Participate in at least one agency wide, one program wide, and as many site level activities as assigned.
- Acquire community resources and share with component staff.
- Other duties as assigned.
