Our community support team delivers intensive, community-based services to individuals with complex mental health needs to enhance independence and quality of life.
Job Scope: The CST Coordinator provides leadership, oversight, and direct service delivery within the Community Support Team (CST) program. This position ensures the effective coordination of care for individuals with serious and persistent mental illness through oversight, consultation, and direct clinical work. The CST Coordinator facilitates the delivery of high-quality, recovery-oriented services that promote client stability, independence, and community integration, while also working with CST Manager to maintain compliance with standards across the team.
Primary Responsibilities
- Provide daily leadership and clinical consultation to CST Intensive Case Managers.
- Orient and support in training newly hired staff
- Support team members in implementing treatment plans and maintaining compliance with agency standards.
- Deliver a full range of clinical services including assessment, counseling, treatment planning, crisis intervention, and case management
- Help clients apply for and maintain medical insurance coverage including Medicaid and Medicare.
- Engage in the 24-hour program coverage and emergency phone rotation.
- Conduct group and individual sessions within the community, in-home, and at the center.
- Complete all required clinical assessments (including DLA, LOCUS, and IATPs) and documentation within the agency's timeline
- Conduct monthly compliance checks for CST staff as assigned.
- Ensure completion of 30-day CST reviews and 6-month reassessments.
- Attend all required meetings, trainings, and weekly supervision.
- Maintain current knowledge of agency policies and procedures
- Model and promote agency philosophy, mission, and values.
Performance Requirements
- Demonstrate knowledge of documentation standards and capacity to meet documentation expectations.
- Ability to get clients to participate in individual sessions.
- Ability to assess, diagnose, interpret and integrate both verbal and non-verbal client input during session.
- Ability to clinically “listen” during session.
- Ability to establish rapport with clients and interact constructively with others in the workplace.
- Ability to focus client on individualized treatment plans and goals.
- Give information in a clear and understandable fashion and respond constructively to appropriate client questions and concerns.
- Knowledge of human development across the life cycle, as applicable to the clinical population served.
Education and Experience
- Master’s Degree in human services field
- Minimum of 3 years of clinical experience
- Willingness to work a flexible schedule, if needed
- Valid Illinois driver’s license, insurance, and automobile and willingness to transport clients when necessary
Schedule
- Monday – Friday, 9:00am – 6:00pm, 40 hours/week
