The Social Services Director organizes and manages the social service department. Establishes objectives for the department and the organizational structure for meeting those objectives. Evaluates, plans, and directs the psychosocial care provided to the residents and their families in accordance with the care center- Skilled Nursing Facility
objectives and state/federal regulations.
This social work leader schedule includes;
- 80 hours every two weeks; Full Time

Responsibilities
- Manages the social service department in compliance with the state and federal regulations
- Participates in facility and system-wide initiatives, meetings and events
- Coordinates admissions with the admission team as appropriate
- Ability to provide counseling, information and referral
- Manages resident caseload
- Provides one-to-one and group counseling as appropriate
- Serves as a Discharge Planner
- Assists in Vulnerable Adult investigations and completes necessary documentation and reports
Required Qualifications
- Bachelor's Degree: List – BSW (from an accredited school of social work)
Other License/Certification/Registration: List -Licensed by the State of Minnesota Board of Social Work
- 1-3 years of experience
Preferred Qualifications
- 3-5 years of experience
Two years of experience in LTC
Supervisory, management, and administration skills
Experience in Medical Social Work

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions.
