POSITION SUMMARY

The Community Support Program Social Worker provides case management and direct treatment services to assigned individuals who are diagnosed with serious and persistent mental illness (SPMI) and/or a co-occurring disorder and in need of Community Support Program (CSP) services.


QUALIFICATIONS

  • Serves as member of a multi-disciplinary case management unit serving individuals with serious and persistent mental illness (SPMI) and possible co-occurring disability; adheres to State Statutes, Administrative Code, County and Department of Health and Human Services policies/procedures and social work practice standards; complies with Wisconsin State Statutes Chapters 51, 54, 55, and 63.
  • Identifies full range of client needs through thorough investigation and assessment; differentiates between medical, psychological, vocational, social, familial, and daily living spheres; provides direct treatment services as part of a multi-disciplinary team including symptom management, illness education, development of adaptive coping skills and psychosocial rehabilitation services.
  • Applies expertise in cognitive/behavioral therapies and/or other evidence based therapeutic interventions to determine appropriate treatment method; designs individual's service program and/or recovery plan for assigned cases.
  • Provides support for individuals, family, or significant others in individual, conjoint, and group formats; offers information and support in application for and maintaining benefits programs.
  • Directs knowledge of court processes to assist with court proceedings; consults with legal counsel on appropriate case specific matters; provides information for court petitions; attends court-ordered evaluations and hearings; provides honest and accurate testimony; monitors compliance with orders and Wisconsin State Statutes.
  • Maintains awareness of community resources and support services available for individuals in crisis; educates clients about available community resources and how to independently gain access to needed services; secures, coordinates, and monitors necessary adjunctive services for individuals involving appropriate professional and paraprofessional staff in programs.
  • Follows laws regulating narcotics, medications, and patients' rights of consent; designs medication compliance monitoring systems; implements plan for delivery.
  • Transfers expertise of crisis intervention techniques, such as de-escalation strategies and active listening skills, to provide crisis intervention for individuals.
  • Adheres to strict record-keeping and reporting guidelines and laws in compliance with agency-defined fiscal parameters and State Statutes required timelines; documents client contacts and services; maintains client files and client specific data for reporting purposes; creates authorizations of services for payment per an individual’s recovery plan; prepares other records and reports, as necessary.

  • Establishes and maintains effective working relationships with internal and external partners; serves as advocate for clients within the community; attends and participates in staff meetings, supervision times, and other regularly scheduled meetings.
  • Develops professional knowledge and skills by attending training events, conferences, and workshops.
  • Performs other related duties as required or assigned.

TRAINING AND PROFESSIONAL EXPERIENCE QUALIFICATIONS

  • Bachelor’s degree in behavioral science or related filed with 1,000 hours of supervised post-degree clinical experience with chronically mentally ill persons.

OR

  • Bachelor’s degree in social work or related field with 2,000 hours of supervised post-degree clinical experience with chronically mentally ill persons.
  • Appointment will be conditional upon successful completion of criminal and caregiver background checks.
  • Certified Social Worker or possess or ability to obtain a temporary certificate permitting the use of the title “social worker” with two (2) months of hire and become fully certified within the specified time limits of the temporary certificate and maintain certification thereafter during employment.
  • Must have valid driver’s license, an acceptable driving record, and access to private transportation for work-related duties.
  • Must possess and maintain personal automobile insurance in the amount of $100,000/$300,000.
