Monday-Friday 8:30a-5:00p, every 5th weekend on-call rotation, one late-night during the week (could be until 11p)

Job summary:
This professional Registered/Licensed Nurse is responsible for coordinating nursing care for patients in ECHRS unit by providing a holistic approach to patient needs.
- Responsible for completing MDS assessments on a caseload of patient/resident ensuring all regulatory requirements and deadlines are met.
- Must be able to complete concise, accurate and detailed documentation and chart reviews.
- Must be willing to complete continuing education related to MDS independently as well as scheduled trainings, in-services etc.
- Attends care conference meetings with patients/residents and responsible parties
- Attends all required IDT meetings including but not limited to: Medicare Meeting, Behavior Management, Morning Meeting, PPS, GG, etc.
- Must be flexible with work schedule as there are potential mandates as well as one scheduled weekly late night and every 5th weekend on call expected.
- Responsible for learning rolls of entire CCC team (MDS, Admissions and Discharge Planning) and covering for others on the team as needed
- Other duties as needed
Qualifications:
- MDS and case management experience is preferred but not required.
- Must have the ability to cooperate with others and use tact and patience in dealing with residents, their families and visitors.
- Must be detail oriented and able to multi task.
- Ability to be flexible and be capable of adapting to frequent changes including work hours, daily routine, etc.
- Experience as a general staff nurse, in which administrative and supervisory ability has been demonstrated.
- Graduation from an accredited school of nursing and current registration/ licensing with the State Board of Nursing Examiners
