Eastview Health & Rehabilitation Center is seeking a compassionate, professional RN MDS Coordinator to join our team. As our RN MDS Coordinator, you will play a key role in assessing resident health, collecting and analyzing data, and developing individualized care plans that align with each resident’s needs and expectations. This position is ideal for an experienced nurse who enjoys a combination of clinical and administrative responsibilities, with a focus on quality care and compliance.
This position is 32 hours weekly
Job Requirements:
Registered Nurse (RN) License in WI or Compact License
Previous MDS experience in long-term care is preferred but not required
Strong communication, organization, and analytical skills
Job Responsibilities:
Conduct comprehensive assessments of residents
Develop and update resident care plans
Monitor patient care through assessments, observation, and medical record reviews
Collaborate with clinical teams to ensure accurate documentation and effective care delivery
Create and manage medical codes for billing and recordkeeping
