At Easterseals PORT Health (ESPH), our mission is rooted in empowering individuals and strengthening communities.

We are seeking a compassionate, knowledgeable, and driven ACT Team Lead / Licensed Mental Health Therapist to lead our Assertive Community Treatment Team (ACTT). In this leadership role, you will guide a multidisciplinary team dedicated to delivering person-centered behavioral health services directly in the community while supporting individuals on their path toward recovery, independence, and stability.

This isn’t just a leadership role — you will influence change and shape futures for both the individuals we serve and the clinical professionals you support every day.

Your Role in Our Mission

As an ACT Team Lead / Licensed Mental Health Therapist, you will provide leadership for the ACT Team while maintaining a hands-on role in service delivery. You will oversee daily team operations, provide clinical supervision and mentorship to staff, support quality and compliance standards, and collaborate closely with the Psychiatric Care Provider to ensure individuals receive high-quality, ethical, and recovery-oriented care.

You will also participate in intake assessments, person-centered planning, crisis response, and direct community-based therapeutic services for a select group of individuals receiving ACT services. This role combines leadership, collaboration, and direct impact—allowing you to model best practices while supporting staff development and individual recovery outcomes.

Services are delivered where individuals need them most, including homes, shelters, hospitals, and community settings throughout the region.

What You'll Do

  • Provide clinical and administrative oversight of the ACT Team
  • Supervise, mentor, and support multidisciplinary staff
  • Deliver direct therapeutic interventions and community-based services
  • Participate in referral screenings, assessments, and person-centered treatment planning
  • Support quality assurance, utilization review, and documentation compliance
  • Review and approve service documentation, billing, and timesheets
  • Coordinate peer review and ensure timely incident reporting
  • Assist with onboarding, training, and staff evaluations
  • Maintain fidelity to ACT standards and person-centered care practices

Why Join Us?

As a part of our mission, we help our team members embrace their potential, build resilience, and thrive! You will benefit from ongoing professional growth and development as you work alongside leaders of clinical excellence in mental health and substance use services that care about your success.
