- Valid Registered Nurse License, state of Michigan
- Associates degree or better in Nursing
- Vaild Michigan Drivers License
- Experience working with the I/DD Population
- Provides a nursing health assessment to assist in identifying and resolving client’s health and safety concerns. Monitors effectiveness and side effects of medication, standards of care/treatment plans according to a person centered approach, and makes recommendations as needed.
- Assist in developing and linking to community supports for addressing health care needs in cases of individuals with complex medical needs.
- Provide periodic review concerning health care treatment.
- Monitor and evaluate medical charts, nursing care guidelines, health care plans, treatment and response as outlined in the Person-Centered Plan (PCP).
- Based upon the individual’s PCP, develop the scope, frequency, and duration of nursing services.
- Attend special consultations regarding individuals and their health care concerns per the PCP.
- Administer medication per the PCP.
- Interpret physician’s orders, link with other health care professionals, such as a physician, care provider, schools, workshops, support team members, guardians, parents, hospitals and inpatient care programs, as needed to individuals served.
Salary : $125
