Qualifications
- Must be a QMHP in accordance with Medicaid Provider Manual Guidelines:
- Possess specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) OR one year of experience in treating or working with a person who has mental illness; AND
- Be a human services professional with at least a bachelor’s degree in a human services field
Duties and Responsibilities:
- Responsible for monitoring ADT alerts and making outreach within 72 hours of hospitalization.
- Responsible for coordinating care upon discharge ensuring appointment scheduled and attended 7 days after discharge and ensure continuous engagement for the first 30 days.
- Provides case management services for individuals, families and/or guardians for support, rehabilitation and/or crisis intervention purposes in the home and community. Monitors and evaluates living situations, school and/or program placements of individuals to determine consistency of treatment and progress, as applicable.
- Coordinate with crisis system partners, hospitals, acute care needs and lead efforts on clinical crisis response.
- Performs discharge planning activities in accordance with best practice standards for individuals being released from a hospital setting.
- Assists individuals with medication needs and coordinates with psychiatrist as appropriate based on treatment needs. Provide support, education, and reinforce the use of prescribed medications.
- Demonstrate competency in providing trauma informed crisis intervention services.
