Position Summary:
The Health Home Quality & Education Director is responsible for overseeing quality improvement and compliance initiatives for the Health Home program. This position provides leadership for the Health Home quality team and the Health Home Specialized Health Consultants and ensures the program’s policies and procedures meet organizational guidelines, funder requirements and accreditation standards.
This position fulfills the Health Home Director role per DMH Health Home guidelines.
Essential Functions:
- Creates and revises Health Home policies and procedures as needed, establishes compliance structure and oversight, and coordinates accreditation reviews and funder audits. Provides input on program strategic initiatives.
- Oversees program data management systems. Manages the preparation and maintenance of quality-related reporting. Coordinates the tracking and trending of Health Home quality data.
- Ensures Health Home data and metrics are consistent with program, funder and organizational objectives.
- Oversees the quality enhancement process for internal program audits and reports, including action plans to ensure timely resolution for identified concerns.
- Provides information to leadership regarding additional training topics based on identified trends and ongoing regulatory requirements.
- Manages the Health Home quality team in carrying out program activities. Ensures staffing ratios are met and that team objectives are accomplished in an effective and timely manner. This includes hiring, training, coaching and developing team members.
- Supervises Health Home Specialized Health Consultants, including hiring, coaching and developing team members. Ensures team objectives are accomplished and that specialized trainings are effective through evaluating attendance, pre/post-assessments and surveys.
- Attends and contributes to DMH Health Home meetings to improve processes and services. Serves as a member of the Senior Leadership Team and participates in committees and ad hoc projects as needed.
- Participates in the creation of the program’s annual budget. Responsible for monitoring and making adjustments related to monthly financials for the quality team.

Qualifications:
- Must have bachelor’s degree in human services field (social work, psychology, counseling, etc.).
- Must have at least 3 years of management experience in IDD, behavioral health or medical systems.
- Must have experience using Microsoft Office suite and must have intermediate-level skill with Excel.
- Previous experience working with Electronic Health Record systems or other data systems preferred.
Additional Requirements:
- This position typically works weekday, daytime hours. May work evenings and weekends as need.
- May travel to other locations for meetings. May have overnight travel.
- Must have current driver’s license and proof of insurance.
- Must complete and maintain all required trainings/certifications.
- Must maintain confidential information in accordance with HIPAA regulations.
