ESSENTIAL JOB RESPONSIBILITES:

QUALIFCATIONS:
Work is performed in a single-story residential setting that is environmentally controlled in Monroe, Louisiana.
ESSENTIAL JOB RESPONSIBILITES:
- Assesses for immediate and long-term service needs
- Completion of monthly observation and monitoring.
- Conducts and Completes Treatment Plans.
- Assist residents with navigating social service systems and accessing benefits, including Social Security (SSI/SSDI) and other relevant programs.
- Obtain and maintain SOAR (SSI/SSDI Outreach, Access, and Recovery) certification.
- Planning, scheduling, and coordinating monthly meetings with residents and stakeholders.
- Responding to client changes in needs, completing/ submitting appropriate paperwork.
- Researching, accessing, and linking to services.
- Communicating and working with providers to ensure appropriate service provision.
- Meeting with supervisor for weekly supervision and consultation.
- Attending/Participating in staff meetings with RA’s.
- Assist with formal and informal training of residential staff according to policy and funding source as needed.
- Monitor the quality of residential programs through established reviews.
- Complete application for social security and other benefits.
I. PROVISION FOR CASE MANAGEMENT SERVICES:
- Assesses and addresses in ISRP/needs expressed.
- Make referrals to appropriate agencies and/or providers.
- Documents and follows up in a timely manner to ensure services are in place and appropriate
- Establish ongoing rapport with clients and assists them as appropriate in maintaining community living.
- Responds to client referrals and needs in a timely manner.
- Empower clients and families to be active participants in their care.
- Works with the client, service providers and families in setting long term and short term goals for the client; link services to meet these goals; and monitors progress of the client achieving set goals.
- Follow up on all benefits applied for the residents
- Keep accurate and up-to-date records and documentation.
- Completes Comprehensive Plan of Care and quarterly reviews, amendments and other paperwork.
- Maintains thorough, up-to-date progress reports.
IV. STAFF DEVELOPMENT
- Work with the Residential Supervisor to ensure maximum communication and coordination with other Easterseals staff, e.g., administrative assistants, other program development staff, etc.
- Work with Residential Supervisor and RA’s in evaluating program effectiveness and compliance within State/Contract requirements and program goals.
- Participate in training activities and programs as required by funding source, policy, and supervisor.
- Participate in trainings for certifications, credentials or specialties as determined appropriate by Easterseals Louisiana, funding source or supervisor.
- Provides input and assistance to Residential Supervisor in the preparation of program reports.
- Problem Solving/Decision Making – Assess needs and consider course of action to best meet those needs.
- Monitoring – Monitor participant progress and respond to unmet needs; able to monitor provider service provision and billing practices and to ensure accuracy and efficiency.
- Planning – Establish effective objectives, priorities, and implement plans; anticipate obstacles and consider alternatives.
- Coordination – Make adjustments in casework to coordinate efforts with other agencies to ensure consistency and adequate follow-up.
- Action Orientation – Take initiative rather than passively accepting; takes action and attempts to influence events to achieve goals.
- Work Commitment – Willingly commits extra effort and time to get a job done; identifies with organization goals; exhibits excellent time management abilities.
- Oral and Written Communication Skills – Ability to exhibit appropriate use of grammar and spelling in written communication, exhibit appropriate use of grammar in oral communication; ensure oral and written communication is professional.
- Building Relationships – Able to establish rapport with provider agencies and staff, Easterseals administrative and other program staff.
- Team Playing – Work cooperatively with other Easterseals Residential Staff/Supervisors.
- Mission Advocacy – Ability to identify with project and Easterseals objectives; adequately, constructively and directly communicate these objectives.
QUALIFCATIONS:
- Must have a bachelor’s degree in social work or human services-related field and one year experience in the social service field.
- Must have own transportation.
- Must have excellent written and verbal communication skills.
- Must be a team player.
- Easterseals Louisiana is a Drug-Free Workplace, pre-employment and random drug tests will be administered.
- Will be required to pass Easterseals Louisiana background check.
- The employee is regularly required to operate a computer, file and retrieve written documents, communicate with others on the phone and in-person.
- The employee is frequently required to walk, sit, use hands, and lift and/or move lightweight items.
- Ability to move independently within the facility and community.
- Must be able to travel and meet with participants
