Position Overview
The Intake Specialist serves as the first point of contact for veterans and their family members seeking services through the program. This position is responsible for conducting intake assessments, determining eligibility, collecting required documentation, and guiding participants through the enrollment process. The Intake Specialist provides information, referrals, and resource navigation services while maintaining accurate client records and ensuring compliance with program and funding requirements.
This position works closely with program staff, community partners, and referral sources to ensure coordinated service delivery and a positive client experience.
Essential Duties and Responsibilities
Client Intake & Enrollment
- Serve as the first point of contact for veterans and family members seeking services.
- Respond to referrals, inquiries, and requests for assistance in a timely and professional manner.
- Conduct intake assessments and eligibility screenings.
- Collect, review, and process required documentation for enrollment.
- Guide clients through the intake and enrollment process while providing clear communication and support.
Client Support & Resource Navigation
- Provide information about available services and program requirements.
- Connect eligible participants with appropriate internal services and resources.
- Offer referrals and resource navigation for individuals who may benefit from community-based services.
- Maintain a client-centered approach while supporting veterans and their families.
Documentation & Compliance
- Maintain accurate, complete, and timely client records.
- Enter and update client information in electronic data management systems.
- Ensure documentation complies with agency, program, and funding requirements.
- Protect client confidentiality and maintain professional standards at all times.
Collaboration & Program Support
- Collaborate with program staff, referral partners, and community organizations to coordinate services.
- Participate in team meetings, trainings, and professional development opportunities.
- Assist with program operations, outreach activities, special events, and community engagement efforts.
- Perform other duties as assigned to support program goals and client needs.
Participate in required meetings and organizational activities, including:
- Regular check-in meetings with the Program Manager as scheduled.
- Monthly in-person Veterans Program team meetings.
- Agency-wide General Staff Meetings, conducted virtually and in person on a rotating basis.
- Required trainings, professional development opportunities, and program-related meetings as assigned.
Required Qualifications
- High School Diploma or GED equivalent.
- Minimum of one (1) year of experience in customer service, client services, administrative support, healthcare, social services, or a related field preferred.
- Strong organizational skills with exceptional attention to detail and follow-through.
- Excellent verbal and written communication skills.
- Ability to communicate professionally and compassionately with veterans, family members, and community partners.
- Demonstrated ability to manage multiple priorities, deadlines, and documentation requirements in a fast-paced environment.
- Proficiency with Microsoft Office Suite, including Word, Excel, and Outlook.
- Experience working with electronic databases, record management systems, or data entry platforms.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Commitment to providing high-quality customer service and maintaining confidentiality.
- Ability to work a hybrid schedule and travel throughout Houston and surrounding areas as needed.
Preferred Qualifications
- Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Healthcare Administration, or a related field.
- Experience working with veterans, military families, or community-based service programs.
- Experience conducting intake assessments, eligibility screenings, client services coordination, or case management support.
- Familiarity with grant-funded programs and documentation requirements.
- Bilingual proficiency in English and Spanish (written and spoken).
