What are the responsibilities and job description for the In Home Coordinator position at Easterseals Central & Southeast Ohio Inc?
PURPOSE: Responsible for daily operations of In-Home Services Program, supervision of Direct Service Professional staff, maintaining budget, ensuring health and safety, and appropriate program development for program recipients. Provide direct care and training to individuals served.
ESSENTIAL FUNCTIONS:
1. Participate in the individual’s Individual Service Plan (ISP) meetings when requested by individual’s interdisciplinary team.
2. Perform tasks and duties according to the ISP; prioritize and organize tasks to achieve outcomes outlined on the ISP.
3. Maintain a clean and safe environment and be sensitive to the individuals and family’s needs.
4. Recognize changes in the individual’s condition and behavior as well as safety and sanitation hazards; report them to the family and case management as appropriate.
5. Provide homemaker/personal care services as needed pursuant to individual’s plan. Weekly direct care service hours requirement will be based on case load and direction of supervisor.
6. Monitor usage of service hours to achieve full billing capacity.
7. Maintain client confidentiality as required by County DD standards and in compliance with HIPAA regulations.
8. Prepare and review individuals documentation for compliance/regulatory standards.
9. Conduct individuals home visits and notations as designed.
10. Assist in coordinating the intake function:
- Screen telephone inquiries for potential clients, conduct potential client interviews and distribute marketing materials, begin new client processing. Provide orientation for new clients, families, and guardians.
- Plan and implement in-service and other staff development activities as appropriate.
- Establish procedures to monitor capacity, availability and initiate placement from waitlist as needed.
- Establish procedures to coordinate billing and fee collection information with fiscal office.
- Ensure all referral, wait list and information calls for the program are documented.
- Ensure all client records are current and complete as required by licenser agents and organizational policies.
11. Responsible for supervision of staff to include:
- Implement evaluation procedures for staff in accordance with established procedures.
- Be responsible for hiring, supervising, training, and terminating staff in accordance with Easterseals, OSHA, FLSA and other applicable labor requirements.
- Provide support to program staff.
- Ensure staff maintains required certification and documentation to meet state and local requirements for employment.
- Oversee administration of direct services to clients enrolled in inhome services program.
- Responsible for scheduling of staff in individuals’ homes
12. Ensure all parties who have dealings and interactions with Easterseals will receive quality customer service.
13. Monitor and approve staff time in Anywhere for payroll and billing.
14. Required to participate in an on-call rotation.
15. Shall maintain MUI/UI log.
16. Maintain a clean and organized work office.
17. Prepare monthly and quarterly reports.
18. Maintain on-line work calendar.
19. Any other duties as assigned by Community Services Manager.
- 1-2 years’ experience in residential care, working with individuals with disabilities and management experience
- Knowledge of Medicaid systems and Individual Options Waivers
- Computer experience, including Microsoft Office Products

- High School Diploma or Equivalent
- 1-4 years of college in a related field is preferred.
- Current CPR/First Aid
- Delegated Nursing Certification
- Valid Ohio Driver’s License – with less than 6 points
- Reliable transportation

PHYSICAL REQUIREMENTS:
- Must be able to lift, carry push or pull a minimum of 50 pounds.
- Frequently requires sitting, standing, walking, bending, squatting, kneeling, and reaching.
- May be required to be on feet hours at a time.
