Job Overview

We are seeking a dedicated and compassionate Case Manager and Counselor to join our dynamic healthcare team. In this vital role, you will coordinate comprehensive patient care plans, provide addiction counseling, and facilitate effective discharge planning. Your expertise will help ensure patients receive personalized support, access community resources, and achieve optimal health outcomes. Duties

  • Conduct thorough patient assessments (ASAM) to determine individual needs, appropriate level of care and develop individualized service plans
  • Provide addiction counseling to patients with behavioral health conditions or substance use disorders
  • Coordinate resident and outpatient services, ensuring seamless transitions across healthcare settings
  • Collaborate with community partners, and facilitate discharge planning
  • Engage with patients and their families to deliver client-centered communication, education, and support throughout treatment journey
  • Manage electronic health records (EHR) and ensure compliance with HIPAA regulations and 42 CFR pt 2, ensuring patient confidentiality
  • Connect patients with community resources such as social and peer services, behavioral health programs, and housing supports

Requirements

  • Current certification CADC-R, CADC-I, CADC-II, or CADC-III or must register within 30 days of hire and become certified in Oregon
  • Knowledge of substance use disorders, recovery principles, and evidence-based treatment practices
  • Ability to conduct or assist with SUD assessments, treatment planning, individual counseling, group counseling, case management, and care coordination within the scope of the employee’s credential
  • Ability to maintain timely, accurate, and professional clinical documentation
  • Understanding of person-centered, trauma-informed, culturally responsive, and recovery-oriented care
  • Ability to establish strong therapeutic relationships while maintaining appropriate professional boundaries
  • Comfortable working with individuals experiencing substance use disorders, co-occurring mental health conditions, housing instability, justice-system involvement, and other barriers to recovery
  • Strong communication and interpersonal skills and the ability to work collaboratively as part of a multidisciplinary team
  • Ability to coordinate care with medical providers, behavioral health providers, community partners, and other supports involved in a patient’s care
  • Commitment to providing respectful, nonjudgmental, low-barrier services and meeting patients where they are in their recovery
  • Familiarity with ASAM Criteria and Oregon SUD treatment requirements preferred
  • Basic computer proficiency
  • Must meet applicable Oregon credentialing, background check, and employment requirements

Preferred qualifications

Experience providing outpatient SUD services, facilitating groups, working with medications for opioid use disorder (MOUD), co-occurring disorders, or serving rural and underserved populations is preferred but not required.

Applicants at all credential levels are encouraged to apply. We welcome newer professionals who are developing their skills as well as experienced CADC-I, CADC-II, and CADC-III professionals.

  • Proven experience working in hospital settings, including Level I Trauma Centers or Level II Trauma Centers, with strong ICU or acute care backgrounds
  • Demonstrated expertise in hospital medicine, emergency medicine, or pediatrics, particularly within high-acuity environments such as PICU or OR experience
  • Knowledge of medical terminology, physiology, diagnostic evaluation techniques, and clinical case management practices
  • Experience working with individuals with mental health conditions or substance use disorders; familiarity with addiction counseling preferred
  • Strong skills in patient assessment, task prioritization, client communication, and discharge planning
  • Proficiency in EMR/EHR systems and electronic health records management; understanding of HIPAA standards and 42 CFR pt 2 , ensuring patient privacy
  • Ability to work effectively with diverse cultural backgrounds and individuals with disabilities or complex social needs

Pay: $22.38 - $38.36 per hour
