Community Engagement and System Development Coordinator (North)

GENERAL DEFINITION OF WORK:
The Community Engagement and System Development Coordinator is responsible for strengthening awareness, access, and collaboration across disability, aging, behavioral health, and related service systems within the northern part of District 7 (Black Hawk, Buchanan, Delaware, Dubuque, Linn, Jones, Jackson Counties). This role leads comprehensive education and outreach efforts, represents the organization in community settings, manages resource information, supports districtwide planning and assessment activities, and facilitates disability services system development.
ESSENTIAL FUNCTIONS:
· Complete outreach and educational activities in all communities in the northern part of District 7.
· Engage community partners, stakeholders, and service providers to strengthen referral pathways and identify opportunities to collaboratively improve the disability system.
· Conduct formal and informal assessments to identify gaps and system barriers.
· Support coordination of partners and stakeholders to implement District priorities and system initiatives.
· Ensure program activities comply with applicable state, federal, and organizational standards.
SPECIFIC FUNCTIONS:
· Deliver targeted education and engagement activities to providers, community groups, schools, civic and faith organizations, medical professionals, and other stakeholders. Utilize large and small-group presentations, outreach events, mailings, and other communication methods to increase awareness of EIDA navigation and disability services/supports.
· Serve as a representative of EIDA at meetings, committees, and community forums. Build and maintain partnerships across multiple service systems (aging, disability, behavioral health, children’s, etc.) to promote coordination, improve service delivery, and support shared goals.
· Gather, organize, and disseminate resources available throughout the district to share with Disability Services Navigators (DSNs), individuals and families, providers, stakeholders, and the community.
· Confer with DSNs and management staff to gather information, identify educational opportunities, coordinate outreach events and meeting participation, and share information.
· Monitor service trends, referral patterns, community needs, service gaps and accessibility concerns, and system barriers. Analyze and document data and finding.
· Collaborate with other agencies, systems, and the community to develop and implement strategies to improve services, systems, outcomes, and the lives of individuals living with disabilities.
· Assist with formal district assessments, analysis of data, development of priorities, and creation of assessment reports and the District 7 plan.
· Support the implementation and tracking of district plan activities to ensure progress and timely completion of outcomes.
KNOWLEDGE, SKILLS AND ABILITIES:
· Knowledge of:
· Disability services and supports, including eligibility requirements and funding sources
· Regulatory compliance and documentation standards
· Community resource navigation and stakeholder engagement
· Microsoft Office Suite and case management systems
· Ability to:
· Represent EIDA professionally in diverse environments.
· Communicate effectively, both orally and in writing
· Coordinate and facilitate meetings and events.
· Utilize strong facilitation, coordination, and management skills.
· Organize and prioritize work to meet competing deadlines
· Build collaborative relationships with diverse stakeholders
· Analyze data and make data-informed decisions
· Adapt to evolving program needs and regulatory changes
EDUCATION AND EXPERIENCE:
· Bachelor’s degree in social work, human services, public administration, public health, or related field and experience within disability, behavioral health, aging, or related systems.
OR
5 years’ experience within disabilities, behavioral health, or aging systems and progressive advancement within the organization(s)
· Experience working with community partners, providers, or public systems
· Experience in project coordination, community engagement, data collection, and/or system development preferred.
This is a hybrid position with 3 days in an office or the community in the northern part of the district and up to two days working from home. Evening and weekend hours will be required for outreach and education.
