Key Responsibilities
- Assist the Social Services Director with resident assessments and care planning
- Provide support to residents and families during admission, stay, and discharge
- Help coordinate community resources and services
- Maintain accurate and confidential documentation
- Support resident advocacy and ensure psychosocial needs are met
- Participate in interdisciplinary team meetings
- Associate’s or Bachelor’s degree in Social Work, Human Services, or related field preferred
- Previous experience in long-term care, healthcare, or social services is a plus
