Position Summary
We are seeking a dedicated and knowledgeable Registered Dietitian (RD/RDN) to join our team. In this role, you will be responsible for assessing residents’ nutritional needs, developing individualized care plans, and collaborating with interdisciplinary teams to ensure optimal patient outcomes.

Key Responsibilities
- Conduct comprehensive nutritional assessments for residents/patients
- Develop, implement, and monitor individualized nutrition care plans
- Provide medical nutrition therapy in accordance with physician orders
- Collaborate with physicians, nursing staff, and other healthcare professionals
- Ensure compliance with federal, state, and facility regulations
- Monitor and document residents’ nutritional status and progress
- Participate in care plan meetings and interdisciplinary conferences
- Educate residents, families, and staff on nutrition-related topics
Qualifications
- Registered Dietitian (RD/RDN) credential required
- Current state licensure (or eligibility) required
- Bachelor’s degree in Dietetics, Nutrition, or related field
- Experience in long-term care, skilled nursing, or clinical settings preferred
- Strong communication and organizational skills
- Ability to work independently and as part of a team
