This role combines census development, referral management, and collaboration with clinical and leadership teams to maintain optimal occupancy while upholding regulatory and ethical standards.
Key Responsibilities
Admissions & Census Management
- Manage all aspects of the admissions process from referral to resident arrival
- Review clinical documentation to determine appropriateness for admission
- Coordinate bed availability with nursing, staffing, and administration
- Maintain target census levels in alignment with facility goals
- Develop and maintain relationships with hospitals, case managers, social workers, and referral sources
- Communicate effectively with families regarding services, availability, and the admission process
- Serve as the primary point of contact for prospective residents and referral partners
- Collaborate with the DON, nursing leadership, therapy, and social services to ensure smooth transitions
- Participate in daily or weekly management meetings as needed
- Address admission-related challenges in coordination with leadership
- Ensure all admission paperwork is completed accurately and timely
- Verify insurance, payer sources, and required authorizations
- Maintain compliance with federal, state, and facility regulations
- Provide clear, compassionate communication to residents and families during a stressful transition
- Address concerns promptly and professionally
- Represent Twin Lakes Nursing Home positively within the community
- Previous experience in nursing home admissions or healthcare intake preferred
- Strong understanding of long-term care regulations and payer sources (Medicare, Medicaid, Managed Care)
