What are the responsibilities and job description for the Community Services Supervisor position at Eastern Area Agency on Aging?
Eastern Area Agency on Aging is currently seeking a full-time (34-40 hours/week) Community Services Supervisor. This position is hybrid- partially remote work from home, and partly based in our Brewer office to oversee the Care Coordination team. The Care Coordination team completes intake/assessments, develops person-centered care plans, provides educational information, links the caregiver and their care partner to services and programs, and provides ongoing support and triage. They also provide information and assistance to older adults and adults with disabilities regarding state and federal services, benefit programs, and community resources.
The Supervisor will perform Care Coordination tasks as well as coach, evaluate, and train the team to ensure clear communication, a team environment, and follow through of policies and procedures. The supervisor will be the expert in the field and can efficiently manage daily operations and fill for any position on their team as a result.
The successful candidate must have exceptional communication and organizational skills, the ability to maintain confidentiality, and handle a variety of tasks simultaneously. Candidates must have the ability to travel for job related requirements.
Bachelor's Degree in Gerontology, Health Services, Social Work, Human Services, and three years of experience supervising staff is required. Licensed Social Worker (LSW) or LSW eligible preferred.
