Location: Knoxville, Tn.
Employment Status: Full-time
Hourly rate: $20.00
Duties/Responsibilities:
- Complete in-home assessments to determine eligibility for the Options for Community Living and National Family Caregiver Support programs to help individual remain independent in the community.
- Develop care plans based on client needs.
- Provide care support services to clients after enrollment.
- Monitor and update care plans over time to best serve the client.
- Enter care support data in a dedicated client tracking system.

Qualifications/Requirements:
- Graduation from an accredited four year college with a B.S. Degree in Social Work, Psychology, Gerontology, Sociology, Counseling, Nursing, or related field; or
- Graduation from an accredited four year college with a B.S. Degree not in a related field and with a minimum of 2 years experience working with older persons and/or adults with disabilities; or
- Minimum of completion of 2 years of accredited college or university and 2 years experience in the field of social work or related field.
- Must have good organizational, interpersonal and decision making skills; or
- Any equivalent combination of education and/or experience.
- Valid Tennessee Driver license.
