Description

Our Family Healing Program provides psycho-educational classes and group support to both men and women who have been identified as using abusive behaviors within their home or community setting. Clients are referred to the program from probation and child protection community partners, primarily in Hennepin county. The case manager works closely with new and existing clients to initiate and increase their involvement in programming, while providing support, advocacy and referrals to community resources to address unmet needs. This position is a primary point of contact for new and existing clients and co-facilitates 1-2 groups per week with another member of Family Healing staff.  

Responsibilities: 

1. Case Management – 55% 

  • Point-of-contact for new clients- receive referrals, walk with clients through the enrollment process, tracking orientation documents, session attendance, and addressing any immediate needs or barriers to success identified including Social Determinants of Health. 

  • Facilitate up to 3 virtual orientation sessions each week for new participants 

  • Establish and maintain effective and professional working relationships with youth (18-24yo) participants throughout their involvement in the program.  

  • Monitor participation within programming.  

  • Maintain a case load of approximately 50 adult clients.  

2. Documentation and Record Keeping – 20%: 

  • Maintain and complete all necessary forms in client files as outlined in Family Healing Program 

  • Complete regular data entry into the Client Track database systems of client forms, program progress, and individual contacts.  

3.  Community Collaboration and Referrals – 15% 

  • Build an extensive network of supportive services and programs to assist participants in the accomplishment of their goals, including: mental health, housing, healthcare, out-of-school time youth programs, etc.  

  • Gain familiarity and linkages with available support at ESNS.  

4.  Group Facilitation – 10%  

  • Facilitate 1-2 weekly DV Intervention groups, providing psychoeducation and process with group members on week’s topic and facilitate discussion surrounding the topic and client’s experiences.  

  • Respond to the different stages of change that the clients are in, as they are all at different stages in the group.  

  • Work with the Family Healing Program Manager and Facilitators to continue the development of the Family Healing Program, based on the client needs and barriers seen in group sessions.  

Other responsibilities as assigned by the Program Manager. 

Qualifications


Education: 

  • Bachelor’s Degree in relevant field such as Social Work, Education, Psychology, or Child Development. [Required per state regulations] 

Experience: 

  • At least one year of related experience providing intake, assessment, and case management services to clients experiencing systemic difficulties. 

  • Experience and understanding of working with domestic violence and victimization and associated professional standards and practices. 

  • Successful experience providing remote or in person based resources. 

  • Demonstrated ability to work independently in an off-site setting, manage time effectively, maintain service guidelines and utilize supervision and support. 

  • Familiarity with Microsoft Suite, Google Suite, Zoom Meeting software, other general computer skills 

Working Conditions/Other Requirements: 

  • Must be available to work evenings, especially for the first month to observe programming (After training, generally until 6:00PM Mondays, possibility for other evening hours as required by programming and client needs) 

  • Must be available to work Monday-Friday daytime hours, some flexibility here but must be discussed with program manager.   

  • Must be comfortable working with clients who have perpetrated Domestic Violence.  

  • Ability to work across ethnic/racial/economic lines to provide effective communication and establish trust with clients. 

  • Clear and assertive communication skills, in both oral and written forms.  

  • Ability to work in a collaborative and coordinated effort with other service providers.  

  • Ability to self-start, effectively problem-solve, and follow through to completion in a timely manner.  

  • Ability to set limits on personal and professional boundaries.  

  • Ability to handle confidential information appropriately and legally. 

  • Ability to relate with and provide outreach to all segments of a multi-cultural community, including under-resourced or under-served clients. The Family Healing Program recognizes that Domestic Violence does not discriminate among those who are impacted, therefore implementing trauma-informed care and advocating for diverse services that promote equality, regardless of gender and sexual orientation, is a crucial aspect when responding to clients who have perpetrated and/or are survivors of Domestic Violence.  

  • Engage with professionalism, mutual respect, and compassion, with other systems’ representatives, including the Family Healing Team and interns, other Agency staff, and clients. 

  • Hybrid Expectation: 80% remote, 20% in office/community meeting clients as necessary 

Salary : $48,000 - $56,000
