The I/DD Case Manager focuses on activities that establish a life-long, person-centered, goal-oriented process for coordinating a range of services, instruction, and assistance that is needed by persons with developmental disabilities. The I/DD Case Manager is responsible for ensuring accessibility, accountability, and continuity of support and services
Duties performed but not limited to the following:
- Adheres to and promotes EastRidge Health Systems mission and values.
- Provides ongoing assessment of an individual’s needs, strengths, weaknesses, available resources and goals.
- Develops and regularly reviews a service plan (Individual Program Plan – IPP) that incorporates information obtained through assessment and knowledge of the existing and needed resources for each individual.
- Provides linkage or referral services to appropriate services to the individuals on caseload.
- Monitors the implementation of the Individual Program Plan to assure that services are being provided as planned, including required monthly face-to-face visits in the residential setting (home visits) and every other month in day program settings.
- Advocates on behalf of the individual to assure accessibility to services and to protect the consumer’s legal and human rights.
- Regularly meets with the client, family members and other relevant people to ascertain strengths and needs.
- Coordinates crisis response planning if needed by the individual.
- Ensures that each participant has freedom of choice to choose among all available service providers.
- Documents all service coordination activities as required by agency policy and procedures and funding source requirements.
- Schedules and convenes IPP meetings and updates, including the timely completion of all relevant assessments that are required, on a consistent basis within established time frames.
- Maintains a minimum productivity level (direct services billed) of 70% per month.
- Remains updated on the important and current trends and changes in the I/DD field.
- Ensures and facilitates service plan development with each assigned client. Plans should be developed according to policy and procedure and meet funding source requirements.
- Consistently meets established compliance standards.
- Ensures that credentialing approvals are met when delivering services.
- Ensures that individuals who receive services are eligible for the services through knowledge of the funding sources and corresponding authorization procedures that are required by the funding sources.
