Title: Case Management Services Manager
Reports to: Director of Programs
FLSA Status: Exempt
Directly Leads: (Total of 6.5 FTE's)
- Case Managers (5 FTE's)
- Substance Abuse Counselor
- Outreach/Health Coach (part-time) (for Residents living with HIV/AIDS)
Budgetary Line Responsibility: Assists EOCP in the development of the agency's budget as it relates to staffing expenses for the programs that this position manages
Salary: $75,000 - $ 80,000
SUMMARY
The Case Management Services Manager provides leadership and coordination of case management and supportive services at EOCP's Crossroads Emergency Housing facility. As such, this position ensures that the delivery of housing case management and supportive services are seamless, responsive to resident needs and adhere to Housing First principles and EOCP's philosophy and strategic direction. The Case Management Services Manager is responsible for assuring that staff utilizes Motivational Interviewing {Ml) as a communication tool to build effective client partnerships. The Case Management Services Manager also makes sure of the proper implementation and integration of supportive services (i.e. money management and substance abuse, and Life Skills training).
ESSENTIAL DUTIES AND RESPONSBILITIES
Development and Management of Case Management Team
- Assesses, develops and monitors effectiveness of case management service procedures, and practices and recommends program enhancements to assure the provision of efficient and high quality services
- Plans and develops a life skills program that support residents in their stabilization plan
- Oversees the development and maintenance of relationships with other providers that support the agency's mission and programs
- Builds and maintains strong client relationships
- Ensures client confidentiality
- Ensures contractual obligations are met
LIFE SKILLS TRAINING
- Responsible for implementing the Life Skills Training curriculum, twice weekly to assure that residents acquire the necessary skills for independent living
- Define roles of case management staff in the implementation of Life Skills Training, and integrate the program into shelter culture and structure.

- Secure consistent attendance of the training module through ongoing outreach and engagement of clients
- Assure that all trainings are interactive and engaging for clients
- Adapt Life Skills training modules to fit the needs of residents
- Administer evaluation surveys after the completion of each training module
SUPERVISION OF STAFF

- Conduct weekly supervisory meetings with Case Managers, Substance Abuse Counselor and part-time Outreach/Health Coach to offer support and to evaluate and develop skills.
- Monitor monthly milestones for all supervisees to promote professional growth
- Conduct progress evaluations
- Maintain consistent communication on a weekly basis with the Director of Programs on the team's progress and any issues/concerns
- Ensure professional needs are addressed by providing individual weekly meetings
CONTRACT MANAGEMENT/REPORTING
- Knowledge of all Crossroads programs and contract requirements
- Develops a matrix that tracks program outcomes and performance measures on a systematic basis ensuring that staff are informed and trained of grant responsibilities and outcomes
- Works consistently with EOCP's Quality Assurance Manager to regularly analyze HMIS reports and performance data
- Ensures that all HMIS data is collected and monitored for timeliness, accuracy, completeness and consistency by case management staff
- Work with the EOCP Quality Assurance Manager to improve internal tracking processes
- Works closely with management to develop effective tracking tools for programs
- Identifies, evaluates, and analyzes issues related to contract management, presents recommendations and develops and implements solutions and processes to improve overall performance for Crossroads programs
- Works collaboratively with the Quality Assurance Manager on funder audits and meets requirements by communicating, evaluating process and provides follow-up to provide recommendations for corrective actions where need be for ongoing compliance for Crossroads programs
- Directs implementation of evaluation tools including surveys, client interviews and focus groups
- Attends county HMIS and funder meetings as needed
- Responsible for reporting to the following funding sources: Community Development Block Grant (CDBG), PATH, FEMA, SSA-CHASS, HOPWA.
Qualifications
REQUIRED QUALIFICATIONS

- B.A. degree in psychology, social work or related field
- Five-years of full-time program management experience in a social service setting managing a case management program
- Five years of supervisory experience with a minimum of five staff
- Five years of contract management experience with responsibilities monitoring grants/contracts
- Proficient in Motivational Interviewing required
- Knowledge of Housing First and Rapid Rehousing preferred
- Demonstrated ability to build and maintain relationships with a wide array of people - of varying ages and from diverse backgrounds
- Maintains and exemplifies outstanding and consistent professional boundaries with clients, staff, and management
- Self-starter, self-disciplined
- Remain focused in the face of pressure, deliver against timelines
- Able to maintain a high level of consciousness and sensitivity to homeless individuals, including individuals who are living with addictions
- Submit to fingerprint clearance
- Provide proof of annual TB test results
Program Planning and Evaluation
- Extensive knowledge of case management techniques
