Family Outreach/Care Coordinator

The Role: The East Longmeadow Public Schools is seeking a compassionate, collaborative, and highly organized Family Outreach & Care Coordinator to support students and families experiencing challenges with school attendance, engagement, and access to school-based supports. Working closely with district school nurses, administrators, the METCO Director, counselors, social workers, and other school personnel, this position will help identify barriers to attendance and support students with complex medical or health-related needs who may require additional coordination and resources to successfully access their education. The coordinator will establish positive relationships with students and families, conduct outreach and follow-up, connect families with appropriate school and community resources, and assist with attendance intervention and care coordination efforts. The position will work collaboratively with the school nursing team to help ensure that students with specialized health needs and their families receive appropriate support and coordination. Strong communication, organization, confidentiality, cultural responsiveness, and the ability to work effectively as part of a multidisciplinary team are essential.
