This is an onsite, Monday-Friday 8am-4:30pm position. NO WEEKENDS or HOLIDAYS!
Candidates must have a valid multi state RN license.

The Patient Care Coordinator is responsible for the delivery of nursing and home health services to the patients served by the Agency, quality of nursing services, effective resource utilization and appropriateness of care. The Patient Care Coordinator provides direction and guidance to home health staff. The Patient Care Coordinator works collaboratively with other leaders in achieving Agency goals.
EDUCATION, EXPERIENCE, TRAINING
1. State Licensure as RN and maintain current in the states of Ohio, WV and PA.
2. Current BLS (AHA) certificate upon hire and maintain current.
3. Minimum of three (3) years nursing experience with two (2) in home health.
4. Minimum one year experience in an applicable setting preferred.
