East House is a dedicated non-profit organization committed to supporting individuals on their journey to recovery. We provide comprehensive services, including residential support, counseling, peer support and community outreach to empower individuals to overcome mental health and substance use disorders and lead fulfilling lives. The Service Coordinator is responsible for partnering with clients in recovery from mental illness, chemical dependency and/or co-occurring conditions to identify, access, and maintain linkages with treatment, housing, rehabilitation, and community services. The goal is to help individuals maintain stable housing in the community and improve their overall quality of life. Working within a person-centered approach, the Service Coordinator teaches clients essential living skills for independent living. 

In compliance with NYS's Pay Transparency Act, the salary range for this position is listed above. East House considers factors such as (but not limited to) scope and responsibilities of the position, candidate's work experience, education/training, key skills, internal peer equity, as well as, market and organizational considerations when extending an offer.

RESPONSIBILITIES 

• Partner with clients to develop individualized service plans based on client-identified goals. 
• Coordinate and advocate for access to mental health, substance use, physical health, financial, and other community resources. 
• Provide support in the community and in client homes, utilizing clinical knowledge and sound judgment. 
• Participate in case reviews and planning to ensure quality and stability of client services. 
• Support client access to housing, including landlord relations, lease retention, and financial assistance. 
• Assist clients with benefit applications, budgeting, and payee agency coordination. 
• Promote family well-being through school, recreation, and healthcare connections. 
• Provide medication support as required by the program. 
• Maintain documentation in accordance with funder and agency standards.

Working Conditions/Work Environment 

Traditional office environment. Occasionally subjected to pressure due to time demands. Travel within the greater Rochester area. Potential for travel outside of Monroe County.

Requirements

Position Qualifications:

Minimum Education & Experience:

• High School Diploma or equivalent with 4-6 years of paid human service experience, or
• Associate's degree with 2-4 years of paid human service experience, or
• Bachelor's degree in a human service-related field with 1-3 years of paid human service experience.

Licensure/Certifications:

• NYS motor vehicle license, safe driving record, and availability of a personal vehicle for work.

Other Skills/Knowledge/Experience:

• Effective counseling with culturally diverse individuals with psychiatric and/or chemical dependency issues.
• Skills in counseling, crisis assessment, and intervention within a psychiatric rehabilitation framework.
• Goal planning and implementation; effective communication and teamwork with diverse clients and staff.
• Ability to assess, monitor, and evaluate needs and progress.
• Basic computer literacy and Microsoft Word skills.
• Experience using multiple database programs.
