Job Summary: This position is responsible for providing a personalized learning environment and a comprehensive system of support to remove barriers to learning and graduation, inside and outside of the classroom.

- Experience working with at-risk youth in an urban setting, required
- Experience working with students with barriers to graduation; preferred
- Experience working effectively with underrepresented populations; preferred
- Experience with de-escalation methods; preferred

Job Duties:
- Assists families with appropriate solutions to barriers presented (i.e., accessing and navigating childcare, transportation, housing, etc.).
- Proactively connects with families when students demonstrate signs of graduation risk factors.
- Travels to homes as needed.
- Documents conversations and attempts on Infinite Campus.
- Relays information from school to family & family to school (i.e., graduation requirements (Core vs. Standard), After Hours, Summer School, Success Street, etc.).
- Connects at least monthly with students to monitor progress.
- Tracks students’ graduation requirements.
- Collects and analyzes data as needed.
- Celebrates students’ success.
- Participate in team meetings to monitor student progress, discuss student concerns and collaborate on curriculum.
- Emphasizes goal setting/decision making and self-improvement skills.
- Provides guidance in students’ selection of careers, academic progress and employability skills.
- Assists with coordinating with outside agencies that can assist students with a variety of needs.
- Performs other duties as assigned.
