Job Overview
We are seeking a dedicated and detail-oriented Care Coordination Representative to join our healthcare team. Duties
- Utilize eClinicalWorks or a comparable EMR system to manage patient information.
- Use Microsoft Excel for data organization and tracking.
- Apply accurate medical and coding knowledge to ensure accurate documentation and reporting.
- Engage with Value Based Care programs through Medicare and Medicaid; utilize and navigate online platforms used by these health plans.
- Perform chart auditing for data retrieval to identify and close care gaps.
- Familiarity with Value Based Care programs and electronic platforms used by health plans.
- Ability to analyze existing Value Based care models for effectiveness, risk level, and eligibility
- Knowledge of HEDIS measures and requirements for closing gaps in care and medication adherence
- Experience in care coordination/care management
- Understand uniform coding methodology and accurately code clinical services provided.
**Qualifications:**
- Must be a Medical Assistant
- Proficiency in eCW or similar EMR systems, Excel, PowerPoint
- Strong attention to detail and organizational skills.
- Ability to quickly learn new skills and tasks as assigned.
