What are the responsibilities and job description for the Case Manager position at East Coast Recovery Center?
The Case Manager is responsible for providing direct client care under the supervision of the Clinical Director. The Case Manager also maintains a safe and efficient working and treatment environment per company policies, procedures and best practices; communicates effectively with the treatment team to ensure safe, quality care is provided to all clients.
JOB SPECIFICATIONS
- Provides direct support services to include engaging clients in case management and personalized aftercare plans.
- Has responsibility for intake documentation. Prepares initial aftercare plan, event notifications and other documentation regarding clients in accordance with requirements, company policies and procedures.
- Monitors caseload daily to ensure required documentation is completed timely; including but not limited to, assessments, case reviews, discharge plans, legal documents, FMLA and short-term disability.
- Facilitates coordination of care to include anything clients require in their initial aftercare plan
- Ability to be resourceful; seek appropriate resources for clients and maintain relationships with outside entities.
- Facilitates psychoeducational groups.
QUALIFICATIONS
- A high school diploma or high school equivalency diploma (GED).
- 1 year of full time behavioral health work experience.
- Clinical onboarding training.
- Knowledge of software programs for personal computer use, especially word processing, email and spreadsheets.
- Knowledge and experience in case management with substance abuse clients.
- Ability to prepare, implement, and analyze aftercare plans.
- Strong problem-solving skills and the ability to carry out assigned projects to their completion.
- Strong interpersonal skills including the ability to communicate effectively both verbally and in writing, establishing, and maintaining effective working relationships, gaining cooperation, and resolving conflicts.
- Ability to plan and organize case workload, manage deadlines of him/herself.
- Ability to maintain and demonstrate ethical professional standards and demeanor, including the confidentiality of employee and client data, client boundaries and other sensitive information.
- Demonstrated competence with and continued commitment to respect for cultural differences and diversity, protecting clients’ rights.
- Recognition of the limits of one’s own professional expertise and the willingness to seek supervision, and/or support as needed.
- Makes clinical and organizational decisions with the clients well being and best interests in mind.
- Recognize, prevent and respond to a situation in which the client may be a danger to himself or others, displaying aggressive behavior or is experiencing a crisis or medical emergency.

Pay: $40,000.00 - $50,000.00 per year
