Nurse RN Care Manager
EBCAP Health Center is seeking a Registered Nurse Care Manager to deliver comprehensive nursing care management services across our health center settings. This role supports patient treatment plans through clinical assessment, care coordination, patient education, and continuity of care working in both office-based and community-based environments.
What you will do
Day-to-day responsibilities in this role
- Deliver nursing care management services aligned with provider orders, patient treatment plans, and regulatory requirements
- Conduct ongoing health assessments to identify patient needs, barriers, risks, and progress; maintain current documentation in the electronic health record
- Coordinate complex care across internal and external providers, pharmacies, laboratories, and community-based services
- Develop, implement, and monitor individualized, problem-focused care plans to support disease management and patient self-management
- Administer, document, and monitor prescribed medications, injections, and treatments; evaluate effectiveness, side effects, and complications
- Perform office-based and community-based nursing services within scope of licensure, including walk-in triage and telephone-based clinical assessments
- Complete accurate and timely documentation of all nursing encounters, including medication administration, reconciliation, vitals, toxicology screenings, and care coordination activities
- Obtain, review, and reconcile clinical information from internal and external medical records
- Facilitate laboratory coordination ensuring tests are ordered, completed, returned, and reviewed; communicate results to providers as appropriate
- Provide disease-specific education to patients on chronic and behavioral health conditions, treatment plans, medications, and symptom management
- Participate in multidisciplinary team meetings, clinical case reviews, and organizational forums
- Promote infection control and patient safety practices in accordance with EBCAP policies, HRSA requirements, and applicable regulations
- Support accurate use of the electronic health record and electronic prescribing systems
- Perform additional nursing duties as assigned to meet operational, clinical, and regulatory needs
What we are looking for
Required credentials and experience
- Associates degree in Nursing required
- Active Registered Nurse (RN) license in good standing issued by the RI Department of Health
- Demonstrated experience in community health, primary care, acute care, urgent care, or behavioral health settings
- Experience coordinating care with interdisciplinary teams, external providers, and community-based resources
