Job Title: Help Me Grow Care Coordinator

Department: Early Childhood

Reports To: Director of Early Childhood Community Services

Employment Type: Part-time

FLSA Status: Non-exempt 

 

Job Summary:

The Care Coordinator serves as the primary staff for the Help Me Grow Centralized Access Point and is responsible for connecting with, providing information to, and linking families of young children to community resources and services that will support and/or enhance their child(ren)’s development.

 

Essential Job Functions   

  • Coordinates the intake of inquiries and referrals received by the HMG Call Line. Inquiries may be related to child development, social-emotional development,behavioral needs, social determinants of health, adverse childhood experiences (ACEs) and other family basic needs.
  • Reviews child/family’s needs and eligibility for programs/services, identifies appropriate referrals, and coordinates referral of child/family to community resources, entitlement systems, and/or other agencies for family support and further assessment.
  • Assists families in understanding and supporting their child’s development and educating them to navigate and advocate for their child(ren).
  • Conducts timely follow-up with families and providers to provide information and assure connection with services.
  • Coordinates the intake of developmental screenings completed by parents/caregivers and received by the Ages and Stages Questionnaire (ASQ) Online.
  • Other duties as assigned. 

Responsibilities and Duties

  • Provides screening questionnaire(s) to parents/caregivers.
  • Tabulates developmental screening results to determine child/family referral needs.
  • Communicates with families by phone to discuss areas of strength and needs as indicated by screening, parent concerns, and/or referring provider concerns.
  • Consults as needed with referring providers regarding concerns.
  • Cultivates and maintains strong interagency relationships with community agencies/resources in order to ensure coordinated linkages to needed services.
  • Enters and maintains data in MiEarly Childhood Connect (MiECC) and ASQ online
  • Travel throughout Ottawa County is required- Must have reliable transportation.
  • Must have flexibility to work evenings to meet the needs of families and the occasional Saturday for community events

Qualifications and Skills

  • Education: Associates degree in early childhood education, social work, social services, or a related field. 
  • Experience: 3 years experience in early childhood, family support services, health services, or community services.
  • Essential skills: Ethical judgement; Innovation and adaptability; professionalism and leadership. Understanding of early childhood system building.
  • Certifications: A commitment to ongoing professional development is essential in this evolving field.

Physical Requirements: 

  • Lifting & Standing: May require lifting (with or without mechanical assistance), climbing, and standing for extended periods.
  • In-Person Presence: Must have the ability to work in-person and engage with colleagues and the public in-person.
  • Mobility & Travel: Required to travel throughout Ottawa County, which necessitates having reliable transportation.

Compensation

  • Compensation: $25.50-$30.00 commensurate with experience.
  • Days: 0.5 FTE of 261 days, year-round. This position is expected to work an average 20 hours per week.
  • Employee Group: non-union hourly.
  • Benefits: Full benefits package, including health, dental, vision, disability, and life insurance. MPSERS provided retirement plan.
