Description: The Family Support Coordinator II (FSC II) will follow and provide support to families who have been identified with developmental concerns and are being connected to early intervention services through Medicaid Part C (the Bay Area Early Steps Program) or Medicaid Part B (Hillsborough County Public Schools FDLRS/Child Find) for evaluation. The FSC II will also follow and provide support to identified families with complex needs who are identified at community screening events. The main activities of the FSC II include initiating / coordinating a home visitation plan, assessing family/child needs, connecting families to referrals and community support services, coaching caregivers and implementing developmentally appropriate activities for the child in their natural environment. These activities are primarily conducted in the family home and/or other appropriate community based settings in Hillsborough County.
Salary:
The starting salary is $47,000.
Education: Bachelor's Degree
Essential Job Functions:
Conduct age appropriate developmental screening using tools such as the Ages and Stages Questionnaire.
Effectively communicate screening results, recommendations and provide developmental guidance to families.
Maintain documentation and case files that are accurate, timely, legible and well written.
Develop kind, supportive and empathic relationships with adult caregivers and young children.
Effectively schedule day to day activities that include phone calls, meetings, community based visits, etc.
Assist with and support Community Developmental Screening events at different locations in Hillsborough County.
Facilitate connection to supports and services in the community for children and families.
