Paraprofessional - Eagle Healthcare Services · Psychosocial Rehabilitation (PSR) · Raleigh, NC

Eagle Healthcare Services, Inc.
Raleigh, NC Full Time
POSTED ON 9/15/2026
AVAILABLE BEFORE 1/12/2027

About the Role

As a Paraprofessional in the Haven pod, you'll work directly with adults living with mental health conditions in a Psychosocial Rehabilitation (PSR) day program. You'll co-facilitate group interventions, provide 1:1 support for daily living skills, award Eagle Bucks (our token-economy system), run the sign-in/sign-out table, and help keep the pod running smoothly.

This position is critical — Haven pod is currently operating with only one PP. Your hire restores the two-PP rotation so neither staff member is solo. You'll partner with another PP and the AP Haven Lead to serve 8 consumers in the Haven pod at a 1:8 ratio.

What You'll Do

  • Co-Facilitate Group Interventions: 30-minute prep before each workshop block; set up the room; support the primary facilitator; take group notes; use redirection and de-escalation (CALMS model); keep transitions ≤10 minutes.
  • Sign-In / Sign-Out Table: Warm greeting; verify consumer identity; guide to the PSR Daily Attendance Google Form; on-time arrivals (by 8:30) earn ClassBank.com bonus points; sign-out 3:00–3:30; zero missed sign-outs.
  • 1:1 Consumer Support: Assist with ADLs, community integration, socialization, communication, problem-solving, budgeting, meal planning, housekeeping, transportation, and wellness routines. Implement assigned interventions per individualized treatment plans.
  • Eagle Bucks Real-Time Award: Observe qualifying behavior; deliver immediate verbal praise naming the specific behavior; credit points in ClassBank.com.
  • Progress Notes: Complete accurate, objective progress notes same shift. Notes require QP co-sign before filing.
  • Observation & Reporting: Promptly report significant changes in mood, behavior, functioning, safety, or medical condition to the AP Pod Lead or on-shift QP.
  • Immediate Escalation: Escalate immediately for medical emergencies, suicidal ideation, aggressive behavior, elopement, medication errors, abuse/neglect allegations, or any safety risk — do not close the loop yourself.
