EAC Network, a leading not-for-profit social service agency dedicated to helping over 80,000 people through 120 impactful programs across Long Island and NYC, is looking for a dedicated full time Clinical Case Manager to join our Nassau County Problem Solving Court Case Management and Recovery Support Services Program!

 

Wage: $32/hour
Schedule: Monday-Friday 9AM-5PM
Location: 99 Main Street Room L88 in Hempstead, NY 

 

What drives the core purpose of this job:

The Clinical Case Manager is responsible for court liaison, assessment, referral and monitoring of dually diagnosed mentally ill and substance abusing clients involved in the criminal justice system into appropriate treatment modalities, management of a caseload, and conducting cognitive behavioral interventions. 

 

What are the key Responsibilities:

  1. Act as liaison to the courts. Conduct assessment interviews and arrange for client to be placed into the appropriate treatment program. Investigate and arrange client service plans.
  2. Complete follow up paperwork and maintain complete client records and have them available for inspection by Supervision at any time. 
  3.  Prepare and present written status reports to the criminal justice system in a timely manner regarding client's progress.
  4.  Investigate and arrange supportive services for clients.    
  5.  Maintain a caseload of substance abusing mentally ill offenders.
  6.  Make site visits to treatment agencies assigned to caseload when necessary.
  7.  Conduct drug testing on clients of the Program as needed.  
  8.  Conduct cognitive behavioral interventions. 

 

What key knowledge, skills, and abilities are required to excel in this role:

  1. Bachelor's degree in psychology, behavioral health, or related mental health field. CASAC or other qualified healthcare credential strongly preferred.
  2. Knowledge of treatment programs and local criminal justice and health systems. 
  3. Good communication skills, both oral and written; Ability to work cooperatively with treatment and  criminal justice systems.
  4. Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
