Job Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) specializing in Long Term Care to join our healthcare team. In this vital role, you will provide comprehensive nursing care to residents in a long-term care facility, ensuring their health, safety, and comfort. The ideal candidate will possess a strong foundation in patient assessment, medication administration, and care planning, with a focus on geriatric and chronic illness management. Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and medication reconciliation
- Administer medications and injections accurately following prescribed protocols while adhering to HIPAA and clinical confidentiality policies
- Monitor residents’ health status, manage acute pain, and respond promptly to emergent situations utilizing triage skills
- Assist with IV infusion therapy, catheterization, wound care, and tube feeding as required
- Document all patient interactions and care activities precisely within electronic health record (EHR) systems such as Epic or Cerner
- Collaborate with interdisciplinary teams to develop and update individualized care plans that address dementia care, Alzheimer's disease, and other memory-related conditions
- Support discharge planning and coordinate with family members or caregivers for ongoing home or community care
- Ensure compliance with long-term care regulations and maintain accurate medical records in accordance with state and federal standards
- Participate in ongoing training related to infectious disease control, hospice care, and acute care practices
Must Have:
- 1 Year Experience required
- LPN License required
- BLS REQUIRED
- LTC LPN 12-hour shifts. More details to be discussed in interview.
