Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) to join our healthcare team. In this vital role, you will provide high-quality nursing care to diverse patient populations across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Duties
- Conduct thorough patient assessments by measuring vital signs, evaluating physical conditions, and documenting findings accurately within electronic health record (EHR) systems.
- Administer medications and injections safely in accordance with physician orders and established protocols.
- Assist with medical procedures such as catheterization, tube feeding, sterile dressing changes, and IV infusion management.
- Monitor patients for signs of acute pain or deterioration, providing timely interventions and triage support.
- Support patients with activities of daily living (ADLs), including mobility assistance, hygiene care, and dementia or Alzheimer's care.
- Collaborate with multidisciplinary teams to develop and implement individualized care plans aligned with long-term care regulations and best practices.
- Maintain strict compliance with HIPAA confidentiality policies and infection control standards.
- Document all patient interactions thoroughly in medical records while ensuring accuracy and completeness.
- Provide education to patients and families regarding medication management, health coaching, and disease-specific care such as geriatrics or pediatric needs.
- Respond promptly to emergency situations using basic life support (BLS) skills and airway management techniques.
MUST HAVE:
- At least two years of SNF/LTC experience - [REQUIRED]
- BLS (AHA)
Shift- Nights (36 hrs guarantee)
Weekend Requirement: Every other weekend
