What are the responsibilities and job description for the RN Care Manager position at E.J.Q. Home Health Care, Inc.?
Assists in the provision of care for home health care clients. Coordinates their plan of care, communicates with the physician's office, Case Manager and others as needed. Provides assessment and skilled services. Performs supervisory visits and coordinated OASIS paperwork for re-certification of services. Work day consists of visits to clients homes in Lorain County and office work to complete paperwork and chart review.
Must be licensed in Ohio, have good time management skills, verbal communication skills and organizational skills. Experience preferred but not required. Must have reliable transportation.

Pay: $28.00 - $32.00 per hour
