Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history reviews to inform care strategies.
- Administer medications, injections, IV infusions, and perform suturing with strict adherence to aseptic techniques and safety protocols.
- Manage complex care procedures such as ventilator support, catheterization, dialysis, tube feeding, and wound care in accordance with established standards.
- Utilize electronic health records (EHR/EHR systems) like Epic or Cerner to document patient information accurately and efficiently.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients across acute care, long-term care, hospice, and behavioral health settings.
- Provide triage assessments in urgent or emergency situations, prioritizing patient needs based on severity and ensuring timely interventions.
- Educate patients and their families on treatment plans, medication administration, disease management—including infectious disease care—and post-discharge instructions.
Skills
- Proven experience working with diverse patient populations including pediatrics, geriatrics, individuals with developmental disabilities, and memory care patients.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), CPT coding, and laboratory procedures.
- Proficiency in EMR/EHR systems such as Epic or Cerner; familiarity with Athenahealth or eClinicalWorks is a plus.
- Expertise in critical care environments including ICU or Level I/II trauma centers; experience in emergency medicine or hospital medicine highly valued.
- Skilled in advanced nursing procedures such as IV insertion and venipuncture; capable of managing acute pain and providing palliative or hospice care.
- Ability to perform diagnostic evaluations including sonography or laboratory specimen collection; knowledge of infection control practices essential.
- Excellent communication skills for effective patient assessment, health coaching, discharge planning, and case management within fast-paced settings.
