Our dedicated, patient-focused Hospice agency is seeking a Hospice LVNorRN Nurse Office Case Manager to join our growing team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients while collaborating with our patient facing field Nurses.

REQUIREMENTS:
- New Grads Welcome
- Valid CA Nursing license
- Candidates must also have excellent customer service
- Interpersonal skills
- Strong problem solving and critical thinking skills
- Ability to thrive in a fast-paced environment
- Excellent computer skills,

- The ability to make extra money after hours and on weekends seeing patients if desired
- Ability to Participate in the weekly On-Call rotation for extra pay
- Working in a patient first environment where our Case Managers are supported by a team of skilled healthcare professionals who are committed to providing the highest level of care.
- Collaborate with Field Nurses to treat, and diagnose patients
- Monitor and report changes in patient symptoms or behavior
- Communicate with collaborating, hospitals, physicians or specialists regarding patient care
- Maintain accurate patient medical records
