Duties
- Assess patients’ physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to determine appropriate treatment strategies.
- Develop and implement individualized care plans that incorporate evidence-based physiotherapy techniques such as trigger point therapy and rehabilitation exercises.
- Deliver hands-on therapy sessions in outpatient clinics, inpatient hospitals, home health visits, and community care settings to promote recovery and functional independence.
- Monitor patient progress regularly, adjusting treatment plans as needed to maximize effectiveness and ensure safety.
- Document all clinical interactions accurately in electronic health records (EHR) systems following clinical documentation standards.
- Coordinate discharge planning by preparing patients for transition to home or community care environments and providing necessary education.
- Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care across acute care, inpatient, outpatient, and home health settings.
- Ensure compliance with medical terminology standards and maintain up-to-date knowledge of orthopedics, neurology, pediatrics, geriatrics, and other specialized areas relevant to physiotherapy practice.
Skills
- Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, or home health environments.
- Strong understanding of physiology and anatomy essential for designing effective treatment plans.
- Excellent patient care skills with the ability to work compassionately with diverse populations including geriatric and pediatric patients.
- Proficiency in medical terminology and clinical documentation standards to ensure accurate record keeping.
- Experience managing electronic health records (EHR) or electronic medical records (EMR) systems efficiently.
- Knowledge of outpatient rehabilitation techniques such as trigger point therapy and orthopedics procedures.
- Ability to perform discharge planning effectively while working within acute care or inpatient settings.
- Familiarity with working in hospital environments or outpatient rehabilitation centers with an emphasis on outpatient experience.
- Skills in working with geriatric patients and understanding their unique needs in home & community care contexts.
- Strong communication skills for collaborating with multidisciplinary teams across various healthcare settings including inpatient hospital experience or outpatient clinics.

Pay: $40.00 - $45.00 per hour
