We are looking for a new team member to join our intensive home visiting services team as a Home Visitor/Case Manager for our Healthy Families Program.
As a home visitor/case manager, you will be responsible for building and sustaining a relationship with a caseload of diverse families who are pregnant or parents of newborns through the child's earliest years. This position requires travel throughout DuPage County. Dual language candidates are encouraged to apply.

- Works with and provides services to young children and their families, by providing weekly home visits at the start of services.
- Outreaches families and builds and establishes trusting, long term (3-5 year) relationships with families.
- Manages a client caseload consistent with department productivity standards.
- Meets with clients in their homes and in the community throughout DuPage County.
- Maintains communication with clients via phone, text, and video visits.
- Assesses family needs.
- Conducts assessments and screenings including parent interview about the family's story and screenings for developmental functioning, depression, substance use, and family violence.
- Connects families with appropriate resources.
