We are looking for a new team member to join our intensive home visiting services team as a Home Visitor/Case Manager for our Healthy Families Program.
As a home visitor/case manager, you will be responsible for building and sustaining a relationship with a caseload of diverse families who are pregnant or parents of newborns through the child's earliest years. This position requires travel throughout DuPage County. Dual language candidates are encouraged to apply.

Responsibilities
Works
Works with and provides services to young children and their families, by providing weekly home visits at the start of services. Outreaches families and builds and establishes trusting, long term (3-5 year) relationships with families. Manages a client caseload consistent with department productivity standards. Meets with clients in their homes and in the community throughout DuPage County. Maintains communication with clients via phone, text, and video visits. Assesses family needs. Conducts assessments and screenings including parent interview about the family's story and screenings for developmental functioning, depression, substance use, and family violence. Connects familieswith appropriate resources.
Salary : $44,400
