Family Health Nurse – Lombard

You will work in a variety of settings including office, clinics, mobile events, and in clients' homes.

Job Details: Monday through Friday from 8:00 am to 4:30 pm with one to two days working until 7:00 pm and on a rotating basis for a total of 37.5 hours a week.

This position includes a paid training program that includes job shadowing, e-learning modules, and structured clinical supervision.

What you'll do as a Family Health Nurse:

  • Provide direct public health nursing services to assigned caseload.
  • Administer health care and nursing services in homes, clinics, schools, daycare facilities, and community settings i.e.: dental screens and fluoride varnish; conducts physical assessments of infants and children.
  • Assist with the training of staff; provides appropriate leadership and direction and promotes positive morale, creativity, and teamwork among staff; delegates work assignments to co-workers as assigned.
  • Educate clients, both individually and in small groups, on a variety of health-related topics including but not limited to nutrition, perinatal care, and breastfeeding.
  • Organize and participate in clinics as assigned.
  • Complete assessments and reassessments of needed health care and social services.
  • Develop individual care plans.
  • Provide appropriate referral information to clients.
  • Counsels' individuals and families with their health needs and concerns.
  • Complete paperwork, medical forms and health histories in a timely manner.
  • Administer and records immunizations.
  • Provide nursing consultations; administers immunizations and screenings for perinatal depression and child development.
  • Present classroom or other community-based presentations.
  • Demonstrate independent judgment and discretion in making position related decisions.
  • Participate in emergency response activities as assigned.
  • Maintain required training, licensure, and/or certification.
  • Maintain confidentiality of privileged information and adheres to patient privacy laws.
  • Demonstrate sensitivity and understanding of other ethnic groups and cultures.
  • Available to respond to clients and/or customers, either in person or over the phone, during assigned business hours and/or clinic times.
  • May serve as clinic or conference charge nurse.
  • May conduct follow-up investigations on reported cases of communicable disease.
  • May provide observational experience for college nursing students, assuming overall responsibility for the shared case.
  • Collaborate with other healthcare professionals to ensure proper coordination of client care.
  • Link clients to needed personal health services to assure the provision of health care needs.
  •  Use formal and informal networks among community organizations to improve the health of individuals, families, and groups within communities.

 

What you'll need:

Completion of a Bachelor’s degree in Nursing and one year of nursing experience; or an equivalent combination of training and experience. Must be trained (within 6 months) as a Competent Professional Authority (CPA) as outlined for the WIC program. 

Attendance at our on-site, paid HR orientation in Wheaton at our Central Public Health Center, is required. 

Good knowledge of practices and principles of public health nursing; good knowledge of social and economic factors influencing client behavior; good knowledge of immunization, diagnostic and laboratory procedures, tests and analyses; good knowledge of the inherent danger and applicable precautions to be taken with the use of medication; good knowledge of local health resources; some knowledge of medical record documentation and maintenance.

Supports, adapts, or delivers services in a manner that ensures understanding and inclusion of diverse client populations. Demonstrates an understanding of the agency’s definition of Integrated Care.
