Job Overview
We are seeking a dedicated and compassionate Registered Nurse with Home Health experience to join our dynamic healthcare team. Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and health history reviews to develop personalized care plans.
- Administer medications via IV infusion, injections, and other routes following strict aseptic techniques and medical documentation standards.
- Manage complex wound care procedures such as suturing, ostomy care, and dressing changes to promote healing and prevent infection.
- Provide specialized care for patients with chronic conditions including cardiac issues, respiratory needs (ventilator management), dialysis support, and memory care.
- Educate patients and their families on disease management, medication adherence, lifestyle modifications, and safety protocols aligned with HIPAA regulations.
- Coordinate discharge planning, case management activities, and collaborate with physicians on diagnostic evaluations and treatment adjustments.
- Utilize electronic health record (EHR) systems like Epic or Cerner to document all patient interactions accurately and efficiently.
Skills
- Proven experience working with individuals with developmental disabilities, dementia care, or in long-term care settings such as nursing homes or assisted living facilities.
- Strong knowledge of medical terminology, ICD coding (ICD-9/ICD-10), and familiarity with EMR/EHR systems including Athenahealth or eClinicalWorks.
- Skilled in critical care procedures including airway management, trauma medicine (Level I & II trauma center experience), ICU experience, and emergency response protocols.
- Competence in performing laboratory procedures like specimen collection/processing and phlebotomy while maintaining infection control standards.
- Ability to handle complex cases involving acute pain management, hospice & palliative medicine, occupational health issues, and behavioral health needs.
- Excellent patient assessment skills combined with physiology knowledge to deliver safe and effective nursing care in diverse environments such as home & community care or outpatient clinics.
- Strong communication skills for triage situations, medical documentation accuracy, and interdisciplinary collaboration.
