- Care Model Design and Execution: Designing, implementing, and executing clinical care models that focus on preventative care and improved patient outcomes across specialties
- Team Management: Managing and training clinical staff, such as nurse case managers and care coordinators, to ensure adherence to new policies and procedures for VBC initiatives.
- Productivity Monitoring: Develops clinical productivity standards and provides coaching and remediation for underperformance
- Performance Monitoring: Utilizing data and analytics to monitor key performance indicators (KPIs), such as hospital admission rates, emergency utilization, in addition to leading metrics around disease progression and appropriate utilization
- Patient Engagement: Overseeing programs and tools designed to enhance patient education, engagement, and care coordination.
- Stakeholder Alignment: Collaborating with physicians and multidisciplinary professional staff to ensure alignment with VBC goals, which may involve providing education and performance management feedback
- Regulatory Compliance: Ensuring that all clinical operations meet regulatory and compliance standards within the evolving healthcare landscape. Proficient in knowledge of current Case Management Standards
- Development: Develops, coaches, and mentors clinical staff including RNs and care coordinators
- Team Composition: Participates in the interviewing and selection process for new hires
- Data Analysis: Review and act on population health dashboards to address care gaps (annual wellness visits, lack of symptom monitoring, missing labs, etc)
- Reporting: Provide regular, timely communication regarding project status, challenges and required actions. Develop tools for documentation of new programs/products as well as monitoring outcomes of these program/products using available data
- Demonstrates applied knowledge of healthcare business processes and extensive experience working with complex, high-acuity patients
- Knowledge of chronic conditions and related workflows, including congestive heart failure, chronic kidney disease, and associated co- morbid conditions
- Evidence of interpersonal skills and a proven ability to affect change and develop and maintain effective relationships across interdisciplinary teams

- Proficiency with navigation of EPIC, web -based applications, Microsoft Outlook, Teams, Excel, Word, and PowerPoint
- Must be able to work and manage clinical staff in a remote environment
- Must be comfortable working in a dynamic, fast-paced environment
- Current Registered Nurse License in the State of Illinois
- Certified Case Manager (CCM) certification preferred
- Current Basic Life Support (BLS) certification is required in this role
- 5 years working in value-based care or strong specialty care experience in cardiac care or kidney disease care
- 5 years combined of related education, experience, or certification.
- Management experience preferred WORK ENVIRONMENT: This position is primarily conducted via telecommunication in a remote setting. The position may require occasional visits to provider offices or in person team meetings with advanced planning. Occasionally visits to off-site locations may be required for training of staff or to retrieve information.
Salary : $95,000 - $145,000
